A person who wishes to become a client
of ACCM must schedule an appointment for an intake with the Case
Manager. An intake is a meeting in which the Case Manager finds
out more about the potential client’s background and needs.
New clients are given an opportunity to talk about their physical
and emotional well-being, as well as their interests in becoming
members of ACCM. This is also a time for prospective clients to
find out more about the organization, as well as its services and
programs.
ACCM clients are comprised individuals
infected with and affected by HIV/AIDS. While the majority of our
services and programs focus on the needs of the HIV+ individual,
partners, families and friends can also receive support from ACCM
by becoming a client. Once an individual has become a client, the
Case Manager is there to follow up on his/her progress and make
sure that the client is aware of resources in the community and
health and social services network. The Case Manager is a good person
to talk to if you’re having problems, can’t find answers
to questions about the virus, or are looking for a specific kind
of service or resource at any time during their membership at ACCM.
